Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted on October 31, 2022. The inspector arrived at 8:50 AM and departed at approximately 11:00am. There were 29 children present and seven staff members. A sample size of 12 records were reviewed which included six children?s records and six staff records. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. The violations are listed on the violation notice issued to the center and were reviewed with the site owner at the exit interview.

Based on record review and interview, it was determined that the vendor did not ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.
Evidence:
1. Staff #5 (date of hire 7/28/22) does not have the Virginia Preservice Training completed.
2. Staff #2 confirmed Staff #5 does not have their Virginia Preservice Training completed.
Plan of Correction: The vendor responded with the following: Staff #1 will ensure Staff #5 complete the Virginia Preservice Training and obtain a copy of the certificate for their record.

Based on record review and interview, it was determined that the vendor did not ensure that within 90 days of the date of employment or 90 days from subsidy vendor approval that staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. Staff #5, (date of hire: 7/28/2022) does not have a CPR certification.
2. Staff #1 confirmed Staff #5 does not have a current CPR certification.
Plan of Correction: The vendor responded with the following: Staff #1 has scheduled staff to complete the CPR and First Aid training for November 12, 2022.
