Inspector Notes

A monitoring inspection was conducted on June 5, 2023. The inspector arrived at 11:50am and departed at approximately 12:45pm. There were 26 children and 7 staff present. The Inspector reviewed two staff records. The inspector observed staff and children?s interactions throughout the inspection and observed ratio and supervision. Children were observed bottle feeding, free play, and nap. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the center?s directors at the exit interview.

Based on record review and interview, it was determined that the center did not ensure that staff complete a central registry finding within 30 days of employment.
Evidence:
1. Staff #2 (date of hire 8/24/22) did not have results of the Virginia Central Registry and no follow up call to obtain the results were conducted.
2. Staff #3 confirmed that there were no results for Staff #2?s Virginia Central Registry result.
Plan of Correction: The center responded with the following: The Virginia Central Registry results will be requested and placed in the record.
Standard 8VAC20-780-160-A
Based on record review and interview, it was determined that the center did not ensure that each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening. The documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #1 (hire date of 06/01/2023) has a tuberculosis screening dated for June 13, 2022, which is outside of the last 30 calendar days of the date of employment.
2. Staff #1 confirmed that they did not have a recent tuberculosis screening this year of 2023.
3. Staff #3, owner, confirmed that there was no recent tuberculosis screening completed for Staff #1 within the last 30 calendar days of the date of employment.
Plan of Correction: The center responded with the following: Staff #1 will complete an updated tuberculosis screening and place the negative results in the case file.
Standard 8VAC20-780-500-B
Based on observation and interview, it was determined that the center did not ensure that a nonabsorbent surface for diapering or changing shall be used.
Evidence:
1. In the infant room, the diaper changing pad had tears to the waterproof lining making it a nonabsorbent surface.
2. The children?s bathroom diaper changing pad, has tears to the waterproof lining making it a nonabsorbent surface.
3. Infant staff confirmed that there were tears to the diaper changing pads.
Plan of Correction: The center responded with the following: The owner will purchase sturdy nonabsorbent diaper changing pads and dispose of the old diaper pads.
