Inspector Notes

An unannounced, on-site monitoring inspection was initiated and concluded on May 20, 2024. The on-site inspection began at 10:15am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff #2 screening was dated July 14, 2021, and Staff #3?s was dated November 22, 2021. Both staff were present and working at the center.
Plan of Correction: Staff will be sent to get a TB test by the end of the week.
Standard 8VAC20-780-245-A
Caregivers shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. Staff #4 did not obtain the required hours in 2023.
Plan of Correction: Staff #4 will complete the required 16 hours of training.
Standard 8VAC20-780-60-A
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction is required. A child with a diagnosed food allergy did not have a written care plan in their file.
Plan of Correction: The center will obtain the written care plan and place it in the file.
