During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on December 17, 2024. The on-site inspection began at 12:10pm and ended at 1:50pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 6 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Staff #1 tuberculosis screening expired on March 31, 2023.
Plan of Correction: The center responded with the following: Staff #1 will obtain a tuberculosis screening this week and place the results in their file.
Standard 8VAC20-780-210-A
Program leaders shall have fulfilled a high school program completion. All staff records reviewed were program leaders that did not have documentation of a high school program completion.
Plan of Correction: The center responded with the following: The program leaders will obtain a copy of their high school diploma and place it in their staff file.
Standard 8VAC20-780-440-E
There shall be at least 12 inches of space between occupied rest mats. In the preschool room, 3 children that were napping did not have 12 inches of space between them.
Plan of Correction: The provider responded with the following: Moving forward, all rest mats will be 12 inches apart.
Standard 8VAC20-780-440-G
Cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat. Child #1 (age 17 months) was observed sleeping in a crib.
Plan of Correction: The provider responded with the following: The staff will obtain a letter from the pediatrician to allow the child to continue to sleep in a crib. Moving forward, staff will ensure that all children above the age of 12 months, will no longer sleep in cribs.
