During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on April 29, 2025. The on-site inspection began at 9:30am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 17 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the
close of business on May 9, 2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1 did not have a copy of their additional immunizations after being at the center for 6 months. Their original immunization record at enrollment were the documents on file.

Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Staff #1 has an expired tuberculosis screening.

Standard 8VAC20-780-245-J-1
To safely perform medication administration practices listed in 8VAC20-780-510, the administration shall be performed by a staff member who has satisfactorily completed a training program for this purpose approved by the Board of Nursing and taught by a registered nurse, licensed practical nurse, nurse practitioner, physician assistant, doctor of medicine or osteopathic medicine, or pharmacist pursuant to ? 54.1- 3408 of the Code of Virginia. Child #1 is diagnosed with a severe food allergy and an emergency medication is prescribed. The center currently does not have any qualified staff members that are Medication Administration Training (MAT) trained and currently possesses the emergency prescription medication at the center.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. Per the center?s medications policy and procedures, it states, ?We will not administer medication at this time.? The center has Child #1?s emergency prescriptive medication in their possession and the instructions on when to administer should the child have an allergic reaction.
