An unannounced inspection was initiated on 09/05/2025 and completed on 10/02/2025 , in response to a complaint received by the licensing office on 08/19/2025 relating to allegations of . supervision, programs and special care provision and emergency. This inspection included document review, tour of the facility, interview, observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
