Inspector Notes

A subsidy health and safety (SHSI) complaint inspection was conducted on10/12/2023. There were 1 child present with the provider supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and a complaint related violation was documented on the violation notice issued to the program. The violation was discussed with the provider during the exit interview.

Based upon provider interview, the provider failed to inform the department's representative of any injury to a child under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence:
According to the provider (vendor), child 1 was injured while in care on 10/02/2023 and was taken to the emergency room. The child has a fractured arm. The provider did not notify the department's representative or complete an online serious injury report.
