A complaint investigation was initiated on 10/12/2023 and concluded on 11/1/2023 . The inspector reviewed compliance in the areas of administration and supervision. One child's record was reviewed. The investigation included interviews, observation and review of documents.
A preponderance of evidence did not support the allegation of inadequate child supervision.
Information gathered during the inspection determined non-compliance with an applicable standard or law and a violation was documented on the violation notice issued to the program.

Based upon investigation, the provider failed to report to the department within 24 hours of the circumstances surrounding the serious injury to a child while under the family day home's supervision.
Evidence:
1. The provider acknowledged that child 1 suffered a broken arm while under the care of the family day home provider. The provider did not notify the department of the child's serious injury requiring a physician's care.
2. The injury was discovered by the department when reported by the complainant.
