Inspector Notes

This inspection was conducted by licensing staff, in part by using an alternate remote protocol, including telephone contacts, documents review and interviews. An in-person tour of the program was also conducted
A Renewal inspection was initiated on 2/1/2022 and concluded on 2/2/2022. The provider was contacted by telephone and a virtual inspection was conducted. The inspector also completed an "in-person" component of the inspection on site. On 2/2/2022 there were 11 children present and 2 caregivers staff during the site inspection. Two staff records and two children?s records were reviewed. The inspector reviewed requirements and compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard 22.1-289.035-B-4
Based on record review and interview, the licensee did not obtain a copy of the results of a criminal history record information check and a sex offender registry check from any state in which a caregiver working in her home has resided in the preceding five years.

Evidence:
The documentation / file for caregiver #2, who was assisting in the care of children on 2/2/2020 during the inspection, did not contain results of a criminal history record information check and a sex offender registry check from the state of Illinois which caregiver #2 documented as a state of residence within the last 5 years on her Sworn Disclosure or Affirmation form. Caregiver #2 was hired 9/13/20219 but had not been working a regular schedule since January 2021. Out of state background checks were required to be obtained for current employees by 12/31/2020.
Plan of Correction: The licensee stated that because caregiver #2 had not been regularly scheduled since January 2021 while enrollment and points were lower, she forgot that she needed to request the out of state background checks from Illinois. The licensee obtained a Sex Offender registry check on line for caregiver #2 during the inspection. The Illinois Criminal Record Check request for Caregiver #2 was sent out on 2/2/2022. The results will be forwarded to the licensing inspector upon receipt by the licensee.
Standard 8VAC20-800-530-G
Based on observation, the licensee did not ensure that children under two years of age do not use pillows or filled comforters.

Evidence:
A toddler whose date of birth is 7/31/2020 was sleeping in a crib with her head resting on u-shaped pillow. There was also a quilted filled unknown fabric object between her head and the pillow. A thick plush blanket was bunched at the end of the crib near her feet. This child is 18 months old and may not use pillows or filled comforters in a family day home until the age of 2 years
Plan of Correction: The licensee stated that she would remove these items from child #3's crib and would not allow children under two years of age the use of pillows or filled comforters going forward.

Based on observation and interview, the licensee did not ensure that children are not allowed to eat or drink while walking, running, playing, lying down, or riding in vehicles.

Evidence:
A n 18 month old toddler (child #3) whose date of birth is 7/31/2020 was sleeping in a crib with a half full bottle of white liquid resting next to her head. Children drinking while lying down is prohibited in family day homes and can pose a choking hazard to the child.
Plan of Correction: The licensee removed the bottle from the crib and will ensure that children are not permitted to bring bottles in their cribs or cots where they will be lying down to rest. The licensee will ensure that children are not permitted to eat or drink while walking, running, playing, lying down, or riding in vehicles.
