Inspector Notes

An unannounced inspection was initiated on 05/22/2025 and completed on 07/17/2025 in response to a complaint received by the licensing office on 05/21/2025 relating to allegations of care of children, preventing of spread of disease, and administration. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to nutrition and physical equipment and environment and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/23/2025. A POC submitted after this date will not appear on the public website.

No soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.
