Inspector Notes
The Licensing Inspector initiated and completed an unannounced renewal inspection on 3/24/2023. The inspector was on site from 1:05pm until 3:15pm.
There were 21 children under the care of 3 staff. The ages of the children ranged from five to eleven years old. The children were observed during outdoor free play and indoor play. Three children?s record, one medical record and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations
4Evidence: Staff #1, with a hire date of 1/25/2019, last TB screening was dated 10/20/2020.
ensure that all climbable playground equipment
had the required resilient surfacing.
Evidence: Observed non-measurable amount
of resilient surfacing under the required fall
zone. The required amount is 6 inches of
resilient surfacing.
Evidence: There was no documentation of emergency drills on record for January, February or March of 2023.
Evidence: Child #1 and Child #2, had no agreement between parents to notify center within 24 hours or the next business day, after the child or any household member has developed a communicable disease.
