An unannounced, on-site monitoring inspection was conducted on 6/24/2024 from 10:45 a.m. to 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were 27 children present and 9 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to do the appropriate background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one previously resided state for Staff 1, who has been employed for more than a month and was working with children at the time of the inspection.
Plan of Correction: Out of state background checks will be conducted as required.

Repeat violation. Medication shall be labeled with the child's name, the name of the medication, the dosage
amount, and the time or times to be given. An unlocked medication in the toddler room was not labeled with the child's name, dosage amount, or times to be given.
Plan of Correction: Medications will be labeled as required.
Standard 8VAC20-780-510-L
Medication shall be kept in a locked place using a safe locking method that prevents access by children. One medication was unlocked in an upper cabinet in the toddler room.
Plan of Correction: Medications will be locked.
Standard 8VAC20-780-560-F-4
Children three years of age or younger may not be offered foods that are considered to be potential choking hazards. A 22 month old child was served raisins at lunch.
Plan of Correction: Choking hazards will not be served to children under the age of 3 years old.
