During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on March 3, 2025. The on-site inspection began at 2:30 p.m. and ended at 5:00 p.m. The inspector reviewed compliance in the areas listed above. There were 39 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.036-A
Background checks must be repeated every five years in accordance with the Code of Virginia. The search of the central registry for Staff 6 was due to be repeated in 2023, but was not repeated until March 2025.
Plan of Correction: The CPS for Staff 6 was repeated March 2025.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of
employment, before coming into contact with children, and shall have been completed within
the last 30 days of the date of employment. Staff 4, employed for 6 months, had documentation of a TB test that was over a year prior to employment.
Plan of Correction: Staff 4 will get a current TB test or screening. Director mistakenly thought the print date was the date the test was given.
Standard 8VAC20-780-510-E
Repeat violation: The center shall ensure that procedures for administering medication shall
include methods to prevent use of outdated medication. The emergency medication for one
child expired 11/2024 and the parent had not been notified that a replacement was needed.
Plan of Correction: A new emergency medication will be requested from the parent. The expired medication will be returned or discarded.
Standard 8VAC20-780-60-A
The facility shall have a written care plan for each child with a diagnosed food allergy, to
include instructions from a physician regarding the food to which the child is allergic and the
steps to be taken in the event of a suspected or confirmed allergic reaction. Child A has
diagnosed food allergies but did not have a written allergy action plan on file.
Plan of Correction: An allergy action plan will be requested from the parent.
