During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was conducted on June 20, 2025. The on-site inspection began at 1:15 p.m. and ended at 3:15 p.m. The inspector reviewed compliance in the areas listed above. There were 28 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 2, 2025. A POC submitted after this date will not appear on the public website.

Fans, when used, shall be out of reach of children and cords shall be secured so as not to create a hazard. The twos room had a fan on a counter; the cord that was plugged in and not secured on the side of the wall, within reach of children.

Over-the- counter medication may be allowed with written authorization from the child's physician and parent. The parent did not renew the written authorization for the medication for Child C every ten working days and physician authorization was not obtained.

When an authorization for medication expires, the facility shall notify parents that that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. The medication authorization for Child A expired two months ago. The authorization had not been renewed and the medication was not returned or destroyed as required. The medication was on site.
