Standard 8VAC20-790-520-F
The center shall maintain, in a way that is accessible to all staff who work with children, a
current written list of all children's allergies, sensitivities, and dietary restrictions. This list
shall be dated and kept confidential in each room or area where children are present. The
allergy list is not kept in a confidential manner and is not dated.

Standard 8VAC20-790-560-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The screenings for Staff #2 and Staff #5 were completed 1 day after hire, the screening for Staff #3 was completed 2 days after hire and Staff #4 was completed 6 weeks after hire.

Staff files shall include documentation that orientation training has been completed. There was no documentation of orientation in 5 out of 5 of the staff (Staff #1-#5) files reviewed. Staff #1 has been working for 3 months, Staff f#2 and Staff #3 have been working for 4 months, Staff #5 and Staff #6 have been working for 5 months.

Standard 8VAC20-790-630-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. Unsafe conditions shall include splintered,cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; protruding nails, bolts, or other components that entangle clothing or skin; and unstable heavy equipment, furniture, or other items that a child could pull down on himself. In classroom #4 there was a tv mounted on the wall and the cord was hanging down and plugged into the wall. The cord poses safety risk to the children that were playing next to the cord.

In areas used by children of preschool age or younger,electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. Two classrooms had electrical outlets without covers within reach of children.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Hazardous chemicals and sanitizer were observed not within reach of children and unlocked, in 5 of the 7 rooms the children were occupying.

A nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing; The diaper pads in 3 rooms were no longer nonabsorbent as there were tears in the plastic cover exposing the foam. .

Disposable diapers shall be disposed in a leak-proof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Four of the classrooms where diaper changing occurs do not have operable foot operated trashcans/ storage systems.
