Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #1 was observed supervising children and had not been obtained the required documentation from a physician or physician's assistant. Staff #'s 1 documentation was overdue by 2 days.

Staff shall be evaluated by a health professional and be issued a statement that the
individual is determined to be free of communicable tuberculosis (TB). Documentation
of the screening shall be submitted at the time of employment and prior to coming into
contact with children. The documentation shall have been completed within the last 30
calendar days of the date of employment and be signed by a physician, physician's
designee, or an official of the local health department.

kept in a locked place using a safe locking method that prevents access by children. Hazardous substances (sanitizing wipes, disinfectant and bleach/ water spray) were observed in an unlocked cabinet in room # 4 and in room #8 a container of hand sanitizer was observed on a desk. These items were accessible to children.
