One staff member to four children from ages zero to 16 months is required. The ratio was not met in room #5 when 5 children were observed with one staff and the youngest child (Child #1) was 15 months.

Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Staff #1 was observed supervising children and had not been obtained the required documentation from a physician or physician's assistant. Staff #'s 1 documentation was overdue by 2 days.

Any person who is expected to be alone with one or more children enrolled in the religious exempt child day center must complete a sworn statement or affirmation before employment at the facility. Staff #4 had been employed for over 12months and does not have a sworn statement.

The vendor shall maintain and keep at the center a record for each enrolled child, which shall be made accessible to the departments representative upon request. The following information was missing
1.The records for Child #1, Child #2 and Child #6 did not include the employment name, address, and telephone number for both parents listed.
2 .The record for Child #3 did not include the written allergy care plan for their diagnosed food allergy and the employment telephone number for one parent listed.
3. The record for Child #4 did not include the employment telephone number for one parent listed
4. The record for Child #7 did not include the home address for the emergency contact listed.

Repeat Violation
Staff shall be evaluated by a health professional and be issued a statement that the
individual is determined to be free of communicable tuberculosis (TB). Documentation
of the screening shall be submitted at the time of employment and prior to coming into
contact with children. The documentation shall have been completed within the last 30
calendar days of the date of employment and be signed by a physician, physician's
designee, or an official of the local health department.
1.Staff f#1 did not have a TB screening and it was overdue by 2 days.
2.Staff #4's TB screening was dated over 2 weeks after employment began.

Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be
kept in a locked place using a safe locking method that prevents access by children. Hazardous substances (sanitizing wipes, disinfectant and bleach/ water spray) were observed in an unlocked cabinet in room # 4 and in room #8 a container of hand sanitizer was observed on a desk. These items were accessible to children.

Repeat Violation
Disposable diapers shall be disposed in a leak-proof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. In room #4 soiled disposable diapers were observed in trash can without a lid and were accessible to children.

Shelter-in-place procedures shall be practiced twice a year. There was no documentation shelter in place drills conducted in 2025.
