Inspector Notes

A complaint inspection was initiated on 11/21/2022 and concluded on 2/10/2022 regarding a complaint that was received by the department on 11/17/2022. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies.
The on site inspection started on 11/28/2022 from10:20 am until 3:25 pm and continued on 12/5/2022 from 1:20 pm until 2:10 pm. Additional interviews and ongoing investigation was conducted outside of the licensed facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interviews and photographic evidence, the licensee did not ensure that staff do not roughly handle children in care.

Evidence:
During an interview with staff #3 on 12/5/2022, staff #3 stated that child #1 (two years old) was climbing on the back of a chair occupied by another child. Staff #3 stated that she "grabbed him by his upper arms and removed him" when the other child began to scoot the chair back. Child #1 sustained a bruise on his upper arm where he was grabbed,

Based on a review of documentation and interviews, the licensee did not always maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

Evidence:
A two year old child (child #1) enrolled for care in the program incurred 4 minor injuries while in care between October 31, 2022 and November 15, 2022.
The Licensee was unable to demonstrate that a written record of child #1's injuries had been documented for any of the 4 injuries.
