Inspector Notes

An unannounced monitoring inspection was conducted on June 17, 2024 from approximately 9:30 am to 12:00 pm. Six staff were present supervising seventeen children ranging in age from one year through school age. Children were observed participating in table activities and eating lunch. Licensing Inspector reviewed standards related to administration, space and equipment, activities, recordkeeping, injury report documentation, inspection reports, emergency supplies and required postings. Five child and six staff records were reviewed. Violations found are noted on the violation notice.

Based on review and interview, facility did not ensure 1 of 6 staff have a completed central registry search within 30 days of beginning employment.

Evidence: The record for staff #5 (start date 12/6/2023) did not have a completed central registry search. Staff member in charge confirmed the central registry search results letter was not available during the record review.
Plan of Correction: Central registry search was received on 6/18/2024.
Standard 8VAC20-780-330-B
Based on observation, facility did not ensure equipment with moving and/or climbing parts on the outside playground have resilient surfacing under and around the equipment within the required fall zones.

Evidence: A four-seater spring rocker was observed on the playground. It was located approximately 4' from the fence when there should be at least 6' of fall zone. There was approximately 2" depth of resilient surfacing under and around this piece of equipment when there should be at least 6" depth.
