Inspector Notes

An unannounced inspection was initiated on February 25, 2025 and completed on April 2, 2025 in response to a complaint received by the licensing office on February 14, 2025 relating to allegations of administration, supervision and programs. The inspector reviewed 1 child?s record on-site. Additional documentation was reviewed electronically. This inspection included document review, tour of the facility and interviews. The preponderance of evidence gathered during the investigation supports the allegation of administration; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 9, 2025. A POC submitted after this date will not appear on the public website.

The center is required to follow the policies and procedures that are required by the standards to include termination polices. The record for child #1 contained a copy of the Child Care Agreement/Contract signed by parent and center. The contract stated a two-week written notice was to be provided if either party terminated services. Parent/guardian was not given notice as stated in the policy.
