Inspector Notes

An unannounced monitoring was conducted on February 7, 2023 from approximately 3:10pm-4:18 pm. There were 53 children present upon arrival with 10 staff members. Additional administrative staff were present. Children were observed eating snack, transitioning from napping and dancing activity.
Three staff records, three children's records . Licensing Inspector reviewed indoor and outdoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did not ensure child has recieved physical exmination 30 days from the child's start date.

Based on record review, the licensee did not ensure there was at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening .

Standard 8VAC20-780-570-A
Based on observation , the licensee did not ensure when a child is placed in an infant seat or high chair, the protective belt was fastened securely.

Evidence: During the inspection, in the infant classroom, a child's straps were not buckeld while sitting in the buckest seat activity table. Confirmed by staff.
