Inspector Notes

An unannounced monitoring inspection was conducted from1:15pm -3:55pm. Records were reviewed remotley. There were 104 children present and 20 staff members present. The sample size consisted of 4 children?s records and 4 staff records. Children were napping and freely playing.

Informatiopl gathered during the inspection determined noncompliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview, the licensee did not ensure there was one staff on duty who obtained with in the last three years instruction in performing daily health observation of children.

The center was unable to provide docuemtnation of one staff during the operating hours with daily health observation training certificate.

Based on observation , the licensee did not ensure areas of the center inside and outside is maintained in a clean safe and operable condition.

Evidence:
In a gym , the facility was using had a bucket of dirty water and their was baseboard peeling off the wall. The staff confirmed observing the area that was not maintained.

Based on observation, the licensee did not ensure that hazordous substances were kept in a lcked place using a safe locking method.

On June 7, 2023, the facility was using a space that had a closet unlocked with multiple chemicals.
