Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on March 11, 2021 and concluded on March 23, 2021. The Youth and Family Director was contacted by email to initiate the inspection. The inspector discussed with the provider a list of items required to complete the inspection. The inspector emailed the Director a list of items required to complete the inspection. The inspector reviewed two children's records, two staff records, and six board officer/agent records submitted by the agency to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the agency.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on document review, the facility failed to ensure that every applicant for licensure as a child day center shall undergo a background check in accordance with The Code of Virginia prior to issuance of a license as a child day center and every five years thereafter.

Evidence:

1. The dates documented on the searches of the Central Registry, in the records for Board Officers/Agents #1, #2, and #3 were dated 03/09/2016. There were no completed repeat searches of the Central Registry for Officers/Agents #1, #2, and #3 available for review. The requests for repeat searches were dated 03/23/2021.
Plan of Correction: "Every Sept. we will review all board member's information including new board."
