Inspector Notes

The Licensing Inspector initiated a monitoring inspection on 12/13/2022 and completed the inspection on 12/28/2022. The inspector was on site on 12/13/2022 from 2:30pm until 3:30pm.

There were thirty-five children under the care of three staff. The ages of the children ranged from five to ten years old. The children were observed on 12/13/13/2022 during homework time, free play time and snack time. Three children?s record, three medical record and two staff records were reviewed on 12/27/2022.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-770-60-B
Based on record review, the center failed to obtain a completed Sworn Statement prior to first day of employment for staff member.

Evidence: Staff # 2 was hired on 3/16/2022 had no documentation of a sworn statement in the record during the inspection. Staff #2 did not provide documentation when requested.
Plan of Correction: Staff will fill out the Sworn Statement on 01/03/2023.

Based on record review, the center failed to obtain a copy of the results of a search of the central registry for any founded complaints of child abuse or neglect within 30 days of employment.

Evidence: Staff #2 was hired on 3/16/2022. A returned search of the central registry was not in the record. Staff #2 did not provide documentation when requested.
Plan of Correction: Staff will complete form for the Central Registry check and form will be sent off.
