Inspector Notes
A subsidy inspection was initiated on 12/13/2022 and completed on 12/28/2022. A licensing inspection was conducted on the same date as the SHSI supplemental inspection, the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Based on record review, the center failed to maintain a current CPR certificate for each staff member within the first 90 days.
