Inspector Notes
A subsidy inspection was initiated and concluded on July 13, 2023. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice and discussed with the center representative during the exit interview.

Based on record review and interview the vendor failed to ensure that staff who work directly with children, in addition to preservice and orientation training annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course.
