Inspector Notes

An unannounced monitoring inspection was conducted Wednesday, August 17, 2022 to determine the center's compliance with licensing standards. The inspector arrived at 10:30 AM and concluded the inspection at 12:55 PM. Today, there were 19 children in attendance with 4 staff supervising. During the inspection, children were playing outside, taking a water break, participating in music and movement, and playing with various materials. Children's equipment, learning materials, indoor and outdoor space, restrooms, and kitchen were reviewed. Medication is administered, and medication authorization forms were reviewed. Four children's records and three staff records, first aid kit, fire drills, and shelter in place drill logs, daily attendance and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Standard 8VAC20-780-270-A
Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: Large areas of cracked and peeling paint were observed in two of the bathroom stalls on each side of the children's toilets.
Plan of Correction: Restrooms will be painted.
Standard 8VAC20-780-510-P
Based record review and an interview, the center did not ensure when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Evidence: 1. The record of child #1 had documentation of a medication authorization form which expired 3/26/2022. 2. The center director stated that they did not notice that the medication authorization form was expired.
Plan of Correction: Will check medication authorization form once a year.
Standard 8VAC20-780-70
Based on record review, 2 of 3 staff records is incomplete. The center did not ensure two of three staff records contained documentation of two or more references before employment. Evidence: 1. The records of staff #2 (employment date: 7/25/2022) and staff #3 (employment date: 7/13/2022) were missing two reference checks.
