During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated and completed on 8/14/2025 in response to a self-report received by the licensing office on 8/4/2025 relating to staffing and supervision, and care and protection. The inspector reviewed 1 children?s record and 2 staff records on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/23/2025. A POC submitted after this date will not appear on the public website.

4Video footage timestamped, August 1, 2025, at approximately 3:33pm, was reviewed on 08/14/2025.
Staff #1 was supervising the three-year-old class during nap time. While children were napping Child #1 was seen on a video putting their hands under Child #2?s shorts near Child #2?s buttocks. Child #2 reported to Staff #2 that Child #1 put their hands under the child?s shorts and tried to pull down Child #1?s underwear. For approximately 10 minutes, Staff #1 was seen on the video sitting approximately 6 feet away in the middle of the doorway. There was a table between her and the children, and Staff #1 did not have site of Child #1 and Child #2. Staff #1 admitted to being distracted by the school age children transitioning to another classroom. Staff #1 also stated that she was looking down at the phone to unlock the front door from staff returning from lunch.
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 8/14/2025, one staff member was observed supervising eight children (youngest child 16 months old) for approximately 15 minutes.
The required ratio for children 4 years old to school age is one staff per ten children.

2. One staff member was observed supervising 11 children (youngest child 4 years) from approximately 11:35am ? 1:10pm in Classroom 3.
