Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated and concluded on 4/14/2021. The director was contacted by telephone to initiate the inspection. There were (56) children present and (14) staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed (3) children?s records and (5) staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of five staff records, the center did not obtain a copy of the results of a sex offender registry check and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Staff #3 (Date of hire 12/21/2020) lived in WV in the last 5 years and the center did not obtain the sex offender registry results prior to the 1st day of hire.
2. Staff #4 (Date of hire 11/30/2020) lived in NY in the last 5 years and the center did not obtain the sex offender registry results prior to the 1st day of hire. The center sent the search of the child abuse and neglect registry or equivalent registry from NY, but did not follow up within 45 days after the request to search was sent.
Plan of Correction: The sex offender registry search was completed today for both staff. I will make sure it is completed prior to hiring someone in the future that lived in another State. I will follow up with NY regarding the search of the child abuse and neglect registry that we never received.
