Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and a virtual tour of the program.

A monitoring inspection was initiated on 8/09/2021 and concluded on 8/11/2021. A self-reported incident was received by the department regarding allegations in the areas of supervision. The director was were contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation and interviewed staff.

The evidence gathered during the investigation supported the self-report of non-compliance with standards or law and violations were issued. Any violations not related to the self-report but identified during the course of the investigation can be found on the violation notice.

Standard 22VAC40-185-340-F
Based on staff interviews, the center failed to ensure that all children under 10 years of age were within actual sight and sound supervision of staff.
Evidence:
1.On 7/12/2021 at approximately 5:00 pm, Child #1 (2-year-old) was left behind in the Two's classroom while Staff #1 transitioned a group of seven 2-year-old children to the playground.
2. Staff #1 stated that during the transition from the classroom to the playground, Staff #1 lined up the 7 children to take them outside. She stated she did the face to name and thought Child #1 (2-year-old) was with her. She was unaware that Child #1 had been left alone in the classroom until Staff #2 brought the child to the playground and told her she left Child #1. Staff #1 stated they had been on the playground approximately 5 minutes.
3. Staff #2 found Child #1 inside the Two's classroom alone at approximately 5:05pm, reported the incident to Staff #4 and took the child to the playground to join their class. Staff #2 stated she told Staff #1 "You forgot her in the classroom."
4. Staff #3 viewed the camera footage and said the child was left alone from approx. 5:00-5:05pm.
Plan of Correction: Staff #1 was re-trained on Face-to-Name Procedures. Staff #1 was re-trained on "Child Abuse and Neglect."
