Inspector Notes
An unannounced monitoring inspection was conducted on 4/18/2022. At the time of entrance, 80 children were in care with 14 staff members present. Children were observed in circle time, completing math work sheets, riding in buggies, playing in centers, listening to the teacher read a story and playing in the brook section of the center. The site was organized and contained developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills, attendance records and medications and allergy action plans were reviewed. 4Evidence:
1. Staff #3 (Date of hire 2/14/2022) did not have documentation that she completed orientation training in first aid and cardiopulmonary resuscitation within 30 days of the 1st day of employment.
Evidence:
1. In the Preschool Room 1 classroom, the paint was peeling to the left of the sink.

1. A container of cereal from home was labeled with a child's name on it and a date from a previous month.

1. Child #4 (Start date 4/04/2022) did not have both parent's work phone numbers and did not have last dates of attendance documented.
