Inspector Notes
An unannounced monitoring inspection was conducted on 9/19/2022 from 10:13am to 1:00pm. At the time of entrance, 62 children were in care with 12 staff members present. Children were observed doing the following: playing at centers, circle time, playing outside, singing songs, playing with dolls, being fed bottles and diaper changes. Interactions between the children and staff were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, the physical space, evacuation drills and attendance records were reviewed. Areas of non-compliance are identified in the violation notice. Violations
4abuse and neglect from any state in which the individual had resided in the preceding five
years and did not request it by the 30th day of employment and did not do the criminal history record check from any state in which the individual had resided in the preceding five
years.
Evidence:
1. Staff #2 (date of hire 11/30/2020) lived in Maryland in the last 5 years and the center did not have record of sending the search of the child abuse and neglect registry to Maryland.
2. Staff # #3 (date of hire 9/16/2019) lived in Alabama in the last 5 years and the center did not have record of sending the search of the child abuse and neglect registry to Alabama and did not have record of obtaining the results of the criminal history record for Alabama.
Evidence:
1. Child #5 had a copy of the physical examination and immunization record, but it was not signed and dated by a physician, his designee, or an official of a local health department.

1. Staff A stated she reviewed the injuries for the year, but did not have written procedures for injury prevention.
