Inspector Notes
The Licensing Inspector conducted an investigation in response to a self-reported incident of a serious injury on 9/22/2022. Interviews were conducted with staff, injury report and staff records were reviewed. The information gathered during this investigation did not result in violations related to the self-reported incident, however the center was cited for one violation unrelated to the incident. Based on review of two staff records, the center did not obtain from each staff member a negative tuberculosis screening within 30 days of hiring.
Evidence:
1. Staff #3 (Date of hire 3/24/2022) had a tuberculosis screening, but it was received after hiring the staff member on 4/21/2022.
