Inspector Notes
An unannounced monitoring inspection was conducted on 5/09/2024 from 9:10am to 11:30am. At the time of entrance, 75 children were in care with 18 staff members present. Children were observed exercising in a circle, reciting the pledge of allegiance, playing in the "Brook" classroom, line up to transition back to their classroom, playing in centers, reciting the Presidents of the United States from flashcard pictures, listening to the teacher read a book, group discussion about a color book in circle time and playing outside. (8) staff records, and (6) children?s records, the physical space, evacuation drills, allergy list, medication, over the counter skin products and attendance records were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Walls and bulletin boards were decorated with children's artwork. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

5Evidence:
1. Staff #1 (date of hire 5/02/2022) had a tuberculosis screening or test dated 5/24/2022.

1. In the Preschool Room 1 classroom, the faucet near the bathroom was loose and the caulking was pulling away from the wall and cracking.
2. In the Brook room, the wallpaper was falling off the wall in an area approximately 12 inches by 12 inches.

1. In the Early Preschool Classroom, the closet door key was accessible and placed inside the lock to the closet. The closet had a disinfecting bleach solution on the top shelf of the closet.
2. In the Infant 1 classroom, a white board care cleaner was inside an unlocked cabinet on the same shelf as eating supplies.
Evidence:
1. In the Two Room, one diaper cream was in a drawer with other diaper creams and was not labeled with the child's name.
Evidence:
1. One child's lunch box in the Early Preschool classroom was not clearly dated and labeled in a way that identifies the owner.
