During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/25/202. The on-site inspection began at 9:10am and ended at 11:31am. The inspector reviewed compliance in the areas listed above. There were 41 children present and 10 staff. The inspector reviewed 4 children?s records and 4 staff records on-site on 11/25/2024.
This inspection included document review, tour of the facility and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/04/2024. A POC submitted after this date will not appear on the public website.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

Staff #2, employed for more than 3 weeks, did not have documentation of requesting a central registry check and did not have documentation of requesting a sex offender registry check from one state. Staff #3 employed for more than 1 month, did not have documentation of requesting a central registry check and did not have documentation of requesting a sex offender registry check from one state.
Plan of Correction: Staff have completed the required background checks needed for out of state. Moving forward, staff that have lived out of State within 5 years will be required to conduct out of state central registry and sex offender searches.

Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine.

Staff A changed a child?s diaper in the infant room and did not wash the child's hands with soap and water before putting the infant down to play after the diaper change.
Plan of Correction: All staff have been retrained on diapering and handwashing procedures.
