During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced inspection was initiated on 3/31/2026 and completed on 3/31/2026 in response to a complaint received by the licensing office on 3/27/2026 relating to allegations of inappropriate behavioral guidance. The inspector reviewed one child?s records and one staff record on-site on 3/31/2026. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/07/2027. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 3/27/2026, Staff 1 was sitting in a chair and grabbed Child 1 (age 2) by their upper right arm as the child was walking away from the carpet area and pulled the child down to the floor and placed the child near their legs.
Plan of Correction: Staff was retrained on positive guidance.
Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 3/27/2026, Staff #1 was sitting in a chair reading a book to the class. Child #1 began walking away from the carpet area and Staff 1 grabbed Child 1 (age 2) by their upper right arm and pulled the child down to the floor and placed the child near their legs.
