During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 03/03/2025 and completed on 03/03/2025, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:44am. The inspector reviewed compliance in the areas listed above. There were 42 children present and ten staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, measurements and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

REPEAT VIOLATION:
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Sanitizer and Lysol were stored on an open shelf above the sink in the diapering area in the infant room, out of reach of children. Lysol wipes were also found in an unlocked cabinet in the preschool classroom, out of reach of children.
Plan of Correction: Moved items on the open shelf to a locked cabinet. And will install child proof locks on the cabinets.

Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.

The annual update of the information in the records of child #1 was overdue by twenty months, child #2 by five months, child #3 by twelve months, and child #4 by seven months.
Plan of Correction: Will obtain parent confirmation that the required information in the child's record is up to date. Have created a form to keep track of annual updates going forward.
