During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/23/2025 and completed on 10/23/2025, as a part of the licensure period. The on-site inspection began at 9:16am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were 35 children present and 8 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child #5, who had been attending for nine months, did not have documentation of an immunization record on file.
Plan of Correction: Will obtain the immunization records from the parents.
Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #5, who had been enrolled for nine months.
Plan of Correction: Will obtain the physical health form from the parents.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials must be kept in a locked place using a safe locking method that prevents access by children.

A can of Lysol was stored on an open shelf in the Orchids room, out of reach of children.
Plan of Correction: Will retrain staff to ensure that all hazardous substances must be stored in a locked cabinet.

Up-to-date information is required to be requested from the parent for each child's record at least annually.

The annual update of the information in the records of child #2 was overdue by 6.5 months and child #3 was overdue by 1 year and 7 months.
Plan of Correction: Will complete the annual update with the parents.
Standard 8VAC20-780-500-B
The diapering area must have a nonabsorbent surface for diapering or changing. For children younger than three years, this surface must be a changing table or countertop
designated for changing. Disposable diapers must be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff
member's hand nor the soiled diaper touches an exterior surface of the storage
system during disposal.

The Tulips classroom for children between the ages 18 months to 3 years did not have a changing table or countertop designated for changing and did not have a foot-operated storage system for the disposal of soiled diapers or pull ups.
Plan of Correction: Will provide a changing table and a foot operated trash can for diapering.
