Inspector Notes

An unannounced renewal inspection was conducted today. There were 2 children (7 points) present with 1 caregiver. The home, children records, caregiver records, household member records, menu's, attendance records, evacuation records, emergency preparedness information, emergency supplies and cribs were reviewed. The children were observed, one was playing with floor toys the other was in a bouncing seat. Areas of non-compliance were cited on the violation notice. Standard 8VAC20-800-100-A
Based on a review of children records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1's record did not contain documentation of a physical examination by the 30th day of attendance. First day of attendance 03/21/2022.
Child #2's record did not contain documentation of a physical examination by the 30th day of attendance. First day of attendance 07/12/2021.
Child #3's record did not contain documentation of a physical examination by the 30th day of attendance. First day of attendance 11/01/2021.
Plan of Correction: I will contact the parents and request physical examination forms.

Based on a review of children records, the provider failed to obtain all required documentation for each child enrolled in the family day home.

Evidence:
Child #2's record does not have documentation of the names of person other than the custodial parents who are authorized to pick up children.
Plan of Correction: I will ask the parents to provide the names of people authorized to pick up the child.
