Inspector Notes

An unannounced, on-site monitoring inspection was completed on 9/10/2024. The on-site inspection began at 12:00pm and ended at 12:55pm. The inspector reviewed compliance in the areas listed above. There were 4 child (9 points) present with the caregiver.
The inspector reviewed 4 child records and 2 caregiver/household member records on-site on 9/10/2024. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were issued. Violations can be found on the Violation Notice.

Standard 8VAC20-800-240-A
Based on observation, areas and furnishings of the family day home, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1. Multiple pieces of the plastic play house in the backyard (outside play area) were broken and on the ground. The broken plastic edges were sharp.
2. The covering over a storage area was broken and had sharp edges.
Plan of Correction: We will remove the sharp toys and fix the storage cover.

Based on a review of children's records, the

Virginia and applicable State Board of Health
regulations.
Evidence:
1. Child #1did not have documentation of updated immunizations on file at the
time of the inspection. The last immunizations documented in the file dated 4/12/2023.
2. Child #3 did not have documentation of updated immunizations on file at the time of the inspection. The last immunizations documented in the file were dated 10/31/2023.
Plan of Correction: I will ask the parents to bring the updated immunizations for the children.
