An unannounced inspection was initiated and completed on 6/17/2025 in response to a complaint received by the licensing office on 5/5/2025 relating to allegations of supervision. This inspection included a tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
