Documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.

The most recent inspection of the child day center is not posted on the licensed premises.
Plan of Correction: The carbon copy of the inspection report had remained posted since the previous inspection. During the most recent inspection, the final report was posted. Moving forward, our administrative team will ensure that the carbon copy is replaced with the finalized inspection report immediately upon receipt to maintain full compliance and accuracy in our postings.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

Staff #1 did not ensure care and protection of the children in the classroom when they were observed sleeping in the classroom while children were in care.
Plan of Correction: Based on our records from the date and time the photo was taken, one additional staff member was present in the room with staff 1 and the infants. While another staff member was present, we understand that this does not lessen the importance of maintaining active supervision at all times, and we take this matter very seriously.

The staff member is being issued a final written warning for failure to maintain active supervision. As a part of the final warning, expectations regarding sight and sound supervision will be reviewed in detail. The staff member was informed that any future violation of supervision will result in further disciplinary action, up to and including termination. The staff member will complete Safe Sleep Training through Safe Sleep Academy.

All classroom educators will complete Safe Sleep Training by 3/6 to reinforce safe sleep standards and supervision expectations. During the Professional Development Day on 2/16, all educators will be retrained on the center's Naptime and Rest Policy.

Administration will conduct unannounced nap time supervision checks in all classrooms moving forward. Observations will be documented and maintained on file. We will address any concerns identified during internal monitoring immediately.

SYSTEMIC DEFICIENCY
Children 10 years of age and older shall be within actual sight and sound supervision of staff.

1. Photographic evidence shows staff #1 sitting on the floor of the Radici (older infant) classroom, asleep, on 10/23/2025 at approximately 11:12 AM. Attendance records indicate that there were 3 infants in the classroom while staff #1 was asleep.
2. Staff #1 was observed sitting in a rocking chair with their eyes closed in the Radici classroom on 2/12/2026 at approximately 2:15 PM. There were 2 infants in the classroom.
Plan of Correction: Reggios Treehouse recognizes the seriousness of this concern. The safety and well-being of the children in our care is our highest priority. Through corrective action, retraining, strengthened monitoring procedures, and reinforced accountability, we are committed to ensuring full compliance with licensing regulations and maintaining a culture of vigilant supervision.

Prepared bottles shall be dated.

A bottle for child #1 was observed in the refrigerator dated 1/8/2026, which was approximately 5 weeks prior. The staff in the classroom stated the bottle was brought in new that morning and the parent had not been updating the date.
Plan of Correction: The staff member observed that the parents dropped the bottle of cows milk upon arrival that morning. The same fay as the inspection (2/12), the previous date label was immediately removed and replaced with the correct date on 2/12. During the Professional Development Day on 2/16, staff were retrained on proper bottle labeling procedures including that all bottles must be dated regardless of the contents (cows milk, breastmilk, formula, etc.). This retraining included the expectation that all bottles and food items are double-checked for the current date upon arrival to the classroom. Additionally, parents were sent a reminder on 2/12 reinforcing the requirement that bottles must be labeled with the correct date prior to arrival each morning. Administration will continue to monitor bottle labeling practiced during routine classroom checks to ensure ongoing compliance.
