Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on 07/26/2021 and concluded on 07/26/2021. There were 33 children present with 6 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and an interview, the center did not ensure to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions.
Evidence:
1. The center did not maintain a written list of children's allergies, sensitivities and dietary restrictions.
2. The record of child #1 (start date: 7/12/21) had a dietary sensitivity listed on the child's registration form.
3. The center representative was not aware of the child's sensitivity.
Plan of Correction: We will create a list of all food and will place them in each classroom.
Standard 22VAC40-665-540-A
Based on record review, the center did not ensure one of three staff records contained documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence:
The record of staff #3 (start date: 4/5/21) had documentation of a negative tuberculosis screening dated 6/21/21.
Plan of Correction: Moving forward we will ensure that TB is completed prior to enrollment but not expired.
