Inspector Notes
An unannounced, on-site monitoring inspection was initiated on November 25, 2024, and completed on November 24, 2024, as a part of the licensure period. The on-site inspection began at 7:50 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were 23 children present and five staff. The inspector reviewed five children's records and five staff records on-site.
This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
31. A plastic child size picnic table on the playground was cracked and missing pieces along the seat.
2. The door frame in the toddler room had exposed chipped drywall accessible to children.
3. A metal carpet strip in the drama playroom was not attached to the floor and could be moved around exposing a sharp edge and posing a tripping hazard.
4. The toilet in the rear bathroom has a connecting bolt that rises above the floor approximately two and a half inches. The bolt did not have a cap on top and posed a hazard of puncturing a child in care.
5. In the infant room, five cribs, all assigned to children in care as young as four months, had oversized and loose-fitting crib sheets on them. The material of the oversized sheets was bunched up and was not tightfitting to meet safe infant sleep requirements.
1. The plastic picnic table was removed from the playground while the inspector was on cite.
2. I will have the handyman repair the drywall around the door.
3. I have already requested the handyman attach the strip with cement nails.
4. I will have the parents provide small crib sheets that will fit the mattresses tightly.
Staff #4, who did not meet lead teacher qualifications, was alone with a group of preschool children. Staff #4 was identified as the lead teach for the classroom.
The emergency drill log for the year 2023, contained documentation of only one shelter-in-place drill.
