The center shall have agents of an applicant for licensure for child day centers to undergo a background check every five years. The most recent sworn statement for applicant #1 expired 20 months ago. The central registry for applicant #1 expired 14 months ago. The national fingerprint background check expired 20 months ago. Updated background checks have not been requested.
Plan of Correction: Per the director: I will have the owner renew the background checks.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Metal braces attached to the wall in the playroom extend outward and pose a risk of snagging the skin of a child in care. The drywall in the toddler room around the door frame is cracked and peeling.
Plan of Correction: Per the director: I will have the landlord come back to fix.

The center shall ensure children under 10 years of age are always within actual sight and sound supervision of staff. Staff #1 left four children, three through four years of age, unsupervised for approximately two minutes, to open the front door to let the inspectors in. Staff #1 did not mention at the time of the inspector?s arrival that they were supervising a group of children.
Plan of Correction: Per the director: I will conduct sight and sound training to remind staff.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days, or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. 1. The medication authorization for child #6 expired one month ago. The medication of child #6 was still on site. 2. The medication authorization for child #7 expired 27 days ago. The medication of child #7 was still on site.
Plan of Correction: Per the director: I will return the medication to the parents to be discarded.
