Inspector Notes

An in-person mandated monitoring inspection was conducted on November 29, 2022 from 1:45pm until 2:20pm. There were no children present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 1 child record, 1 household member record and 1 staff record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Evidence: Child 1 does not have parent work phone numbers, address for 1 emergency contact and is missing a second emergency contact.
