During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 11/24/2025, as a part of the licensure period. The on-site inspection began at 10:50 AM and ended at 11:55 AM. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver. The point total was 12. The inspector reviewed 4 children?s records and 3 caregiver/household members records on-site. This inspection included: document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 12/2/2025. A POC submitted after this date will not appear on the public website.

To safely perform medication administration practices listed in 8VAC20-800-710 whenever
the family day home has agreed to administer prescription medications or nonprescription
medications, the administration shall be performed by a caregiver who has satisfactorily
completed a training program for this purpose developed by or approved by the Board of
Nursing.
The provider's Decision to Administer Medication policy states they will administer
medication; however, the provider has not completed a training program for this purpose,
and cannot administer medications.
Child #2's record was missing the employment information for both parents; and the medical insurance policy number. The child was present during the inspection and had been in care for 3 months.
