Inspector Notes

An unannounced monitoring inspection was initiated and completed on 04/21/2025 in response to a self-report received by the licensing office on 04/10/2025 relating to supervision. The inspector reviewed 1 children?s record and 2 staff records on-site on 04/21/2025. This inspection included document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/28/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On Thursday, April 10th, Child #1 was left on the playground, without supervision, from 12:27 pm to 12:32 pm, following a transition from the playground to the classroom.
Plan of Correction: Staff have completed a re-training of Name to Face; Admin are performing Name to Face audits, and Name to Face signs/reminders have been placed at all thresholds.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

On 04/10/2025, from 12:27 pm to 12:32 pm, Staff #1 and Staff #2 were unaware that they left Child #1 (4 years old) alone and unsupervised on the playground.
