Inspector Notes

A complaint inspection was conducted on April 21, 2023, in response to a complaint that was received by the department on April 17, 2023, regarding allegations in the supervision of the children at the center. Inspector arrived at the center at 10:50am until 12:10pm. Staff were interviewed, and documentation was reviewed. Incident reports were discussed and reviewed. The complaint is determined to be not valid. During the investigation, a violation was found not related to the complaint, and is identified on the attached violation notice.

Based on observation and interview, it was determined that the center did not ensure that the operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
Evidence:
1. The preschool class was observed to transition from the outdoor play area back to their classroom. There were eight (8) children observed coming out of the playground area in which the stipulation to the center?s license is that there shall be no more than six (6) children may be in the outdoor playarea at any time.
2. Staff #1, the owner, confirmed that there were 8 children on the playground and confirmed that the stipulation to the center?s license is that there is only to be 6 children at any time to the outdoor playarea.
Plan of Correction: The center responded with the following: Staff #1 will meet with all staff members and inform them that there can only be six (6) children in the outdoor playarea at any given time.
