Inspector Notes

An unannounced monitoring inspection was completed on 12/04/2024 as a part of the licensure period. The on-site inspection began at 11:15am and ended at 12:35pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 16 children present and 4 staff. The inspector reviewed 3 children?s records and 3 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-3
The center is required to obtain for each staff member a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 for any founded complaint of child abuse or neglect.

The center did not have a copy of a search of the central registry for staff #3, who has been employed for more than 3 months.
Plan of Correction: The owner sent for the background check however has not received results. The owner will follow-up with the Office of Background check Investigations today.

After the first license, annual approval from the health department shall be provided.

The last health inspection conducted at the center was dated 12/07/2022.
Plan of Correction: The owner will contact the health department today and schedule an inspection of the facility.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with
minimum safety standards when tested in accordance with the procedures
described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

On the playground the merry-go-round and small plastic slide did not have the required fall zones surrounding these pieces of equipment.
Plan of Correction: The owner will contact maintenance to remove the small slide to ensure all playground equipment has the required fall zones.

Ground supports shall be covered with materials that protect children from injury.

On the playground, the ground supports of the merry-go-round were exposed and not covered with materials that protect children from injury.
Plan of Correction: The owner will contact maintenance to secure the ground supports of the merry-go-round and ensure they are covered with material to protect children from injury.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

In the infant room there were several prepared bottles of infant formula that were not dated.
Plan of Correction: The bottles are typically labeled with the date, however today the regular teach was out and dating the bottles was missed.
