During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on March 10, 2025. The on-site inspection began at 10:30am and ended at 1:15pm. The inspector reviewed compliance in the areas listed above. There were 20 children present and 4 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Each child shall have a physical examination before the child's attendance or within 30 days after the first day of attendance. Child #1 has been at the center for over a year and there was no documentation of the child?s physical examination in the record.
Plan of Correction: The center responded with the following: The center will obtain the child?s most recent physical and place it in the child?s record.
Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Staff #1 has a tuberculosis test that expired June of 2024.
Plan of Correction: The center responded with the following: Staff #1 will obtain a new tuberculosis test/screening and place the results in their file. Staff #1 will ensure that all staff complete their follow up screening or test every two years.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. In the preschool room, the back of a bookshelf was falling apart with exposed screws and in reach of the children in care.
Plan of Correction: The center responded with the following: Preschool teacher stated that the back of the bookshelf just broke recently. A work order will be put in and tape will be place temporarily on the back of the bookshelf to prevent screws and panels to fall off.
Standard 8VAC20-780-550-D
The center shall implement a monthly practice evacuation drill. The center did not practice an evacuation drill for the month of February 2025.
Plan of Correction: The center responded with the following: Moving forward, the center will ensure that all evacuation drills are completed monthly.
Standard 8VAC20-780-570-A
When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Two infant children did not have their protective belt fastened securely while sitting in a high chair.
Plan of Correction: The center responded with the following: The protective belts were fastened for the two children during the time of the inspection. The center was aware that protective belt shall be fastened when in the high chair and infant seat. Moving forward, all staff will place children in a high chair with the protective belts fastened at all times.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain documentation of viewing proof of the child's identity and age. Child #1 did not have documentation of proof of child?s identity and age.
Plan of Correction: The center responded with the following: The staff will speak with the parent to obtain the child?s birth certificate and obtain a copy for the child?s record.
